LEVITT: So youve devoted much of the last 10 years to understanding longevity better and the things that are under control of people in their behaviors. By implication, it cannot be used to make nutrition recommendations. We certainly didnt learn anything about it in med school. It started very early. Can you describe how you approached your undergraduate classes at Stanford? And I think youve come to appreciate, in recent years, the importance of sleep. I actually find a fasting sleep to be some of the most amazing because Im watching this plummeting temperature, rising heart rate variability, falling heart rate, all of those really valuable things. , I think there [are] basically three ways that we can approach dealing with this: One of these is through psychotherapy, pharmacotherapy, and then behavioral therapy, which Im an overwhelming proponent of in particular a type of behavioral therapy called Dialectical Behavioral Therapy or DBT. , My training is very specific, but now it is fundamentally organized around four pillars: Stability, strength, mitochondrial or aerobic efficiency, and anaerobic performance. , Those four main pillars focus on the different areas Dr. Peter wants to maintain good health even into old age. Because what youre basically saying is: whats the dose? If were going to take Tylenol, we know the dose. My Centenarian Olympics has 18 events in it. And thats really about the highest praise I can offer anyone I know. And lets be honest. It also has a significant effect in terms of fat oxidation. So thats how I ended up at McKinsey. And then the evening would be going to the gym for sparring. Lets watch this thing instead. And shes like, Who is this guy? And I said, Look, I dont agree with half the things this guy says, but he is categorically one of the smartest dudes ever. Peter Attia is a physician focusing on the applied science of longevity. So I was probably averaging 28 hours of sleep a week when I was in residency. And by the way, whatever genes killed everyone before you are coming for you if you dont do something about it. He remains unbiased and sticks to what the science indicates while also willing to experiment on himself. So thats called starvation ketosis. Its also that it has no fiber left in it as well. So I embarked on a sleep experiment that went on for maybe three months where I did not sleep more than three hours a night. And my glucose level is very low. Why werent we immediately engaging in a whole bunch of experiments that would very rapidly get us answers to questions that would enable us to decide what to do? The second thing I would say is: really do not consider sunk costs in your career. From 2012-2015, he . You have to have a second project which youve actually done, but you pretend that you havent done it. And thats important, but just as important is our mental health. Too often, it intervenes with treatments too late to help, prolonging lifespan at the expense of healthspan, or quality of life. How often? The first two are simple: Diet and exercise. Theres one disease, which is not really thought of as a disease, but I think of it as a continuum that is the foundation upon which all of those sit. The problem was, it was the winter. Now, on with the show. I think most people do get a bit of a cognitive jolt from it. Its just so hard to quit stuff. I rarely have a conversation with Peter in which he doesnt introduce me to something Ive never thought about before. And when I was in high school, I became obsessed with him. When we start to experience those mental health issues, we die in a sense. So what are some things we can do to improve our sleep to keep this from happening? It also impairs senescent cells or more likely impairs the secreting factors that are made by senescent cells. And even if there were such data, what data are there that this harm is caused by the glucose itself? And so, to answer your question, Steve, I am still incredibly bearish on sugar. You sometimes need that distance from something. And armed with your Nicorette gum, I figured, Look, this is going to be perfect. The problem is that we may be living longer, but were living with chronic health conditions that significantly lower our quality of life. LEVITT: And I asked you, Well, how do you know when its time to stop working out? You said, Either when I lose consciousness or when I piss or crap myself, thats when I decide Ive worked out hard enough.. Right? in mechanical engineering and applied mathematics. And to me, we want to minimize that gap, right? So this just reinforced in me that, youre not immortal. So maybe some combination of those two things would be helpful as a person navigates their way. So when you contrast living to 70 [and] spending 20 years in misery versus living to 75 with maybe two years in misery, it just doesnt even strike me as a trade-off. . And they have totally different mechanisms. Studying studies: relative risk vs. absolute risk. So theyve either died a cognitive death which is to say their minds have become so dull that theyre really not able to be the people they wanted to be [or] their body has broken down so much that the things that once gave them so much joy, theyre deprived of or emotionally theyve become depressed. The other two preventions, however, might surprise you, If I just took 20 guys like you and took them from eight hours a night to four hours a night for two weeks and then did these glucose tolerance tests, I could reduce [their] glucose disposal by 50%. Now, all of those diseases have different effects and treatment methods, and over the last several decades, weve learned a lot about preventing and reversing them.
